Stephen Ministries is an independent not-for-profit group based in the United States that was organized in oder to supplement pastoral care in congregations. Curriculum and training is available for congregations to equip people to ministry to those that are grieving. One of the resources available is a set of booklets entitled "Journeying Through Grief", these books are designed to be sent to someone that has had a death or loss in four stages, (3 months, 3 months, 6 months, and 11 months). I recommend them for congregations to use for people that are going through grief.
